Platform Participation Acknowledgment

This Platform Participation Acknowledgment explains the role of HUB Healthcare, LLC (“HUB,” “we,” “us,” or “our”) in connection with the HUB platform (the “Platform”). By creating an account, accessing, or using the Platform, you acknowledge and agree to the following.

1. HUB Is a Technology Platform

HUB Healthcare operates a technology platform that allows individuals and organizations to connect, communicate, coordinate services, schedule activities, exchange information, and manage workflows.

HUB does not provide caregiving services, medical services, home care services, healthcare services, or any other professional services. The Platform only provides tools that enable users to connect and coordinate with one another.

2. Services Are Provided by Independent Users

Any services arranged through the Platform are performed by independent individuals, contractors, organizations, or providers who use the Platform.

HUB does not employ, supervise, manage, or control these individuals or organizations. Users of the Platform are responsible for their own services, actions, qualifications, conduct, and compliance with applicable laws.

Nothing in the Platform creates an employer–employee relationship, partnership, joint venture, agency relationship, or franchise relationship between HUB and any user.

3. HUB Does Not Supervise or Direct Services

HUB does not:

supervise or monitor services performed by users;

train or instruct service providers;

control how services are performed;

guarantee the quality, safety, legality, or suitability of services; or

verify that services meet professional or regulatory standards unless explicitly stated.

Users are responsible for evaluating the qualifications, experience, and suitability of individuals or organizations they choose to interact with through the Platform.

4. User Responsibility

Users are responsible for their own decisions, interactions, communications, and agreements made through the Platform.

Users agree that they:

assume responsibility for their interactions with other users;

will exercise appropriate judgment and caution when engaging with others;

will comply with applicable laws, regulations, and licensing requirements; and

will follow all applicable Platform policies.

5. No Healthcare Provider Relationship

HUB Healthcare is not a healthcare provider, medical practice, home care agency, or clinical services provider.

The Platform is not intended to diagnose, treat, monitor, or manage medical conditions. Nothing on the Platform should be interpreted as medical advice, clinical guidance, or professional healthcare services.

If medical attention is required, users should seek assistance from licensed healthcare professionals or emergency services.

6. Platform Role in Transactions

HUB may provide features that support scheduling, messaging, payments, workflow coordination, or record keeping between users.

However, HUB is not a party to the services or agreements between users unless explicitly stated in a separate written agreement.

All service arrangements are made directly between users of the Platform.

7. Assumption of Risk

By using the Platform, you acknowledge that interactions between users occur outside of HUB’s direct control. Users assume the risks associated with meeting, interacting, or coordinating services with other users.

HUB is not responsible for the actions, omissions, conduct, performance, or behavior of users of the Platform.

8. Relationship to Other Agreements

This acknowledgment should be read together with the following documents, which govern your use of the Platform:

Platform Terms of Service

Privacy Policy

Payment and Cancellation Policy

Acceptable Use and Safety Policy

Any additional terms that apply to specific users, roles, or services

If there is any conflict between this acknowledgment and the Terms of Service, the Terms of Service will control.

9. Acknowledgment

By accessing or using the Platform, you acknowledge that you have read, understood, and agree to this Platform Participation Acknowledgment.
