Client and Family Terms

These Client and Family Terms (“Client Terms”) govern the use of the HUB Healthcare platform (the “Platform”) by individuals, families, households, or representatives requesting services (collectively, “Clients”).

These Terms supplement the HUB Platform Terms of Service. By requesting services, creating an account, or otherwise using the Platform as a client, you agree to comply with these Client Terms and the Platform Terms of Service.

1. Platform Role

HUB Healthcare operates a technology platform that helps connect individuals and organizations. HUB does not provide caregiving, home health, nursing, or in‑person services.

By using the Platform, you acknowledge that:

HUB is not a home care agency;

HUB is not a healthcare provider;

HUB does not employ caregivers;

HUB does not supervise services performed by caregivers or providers.

Any services arranged through the Platform are provided by independent providers.

2. Client Responsibilities

Clients are responsible for:

providing accurate information when requesting services;

clearly describing the services needed;

ensuring the service environment is safe;

complying with applicable laws and safety standards;

communicating respectfully with providers.

Clients must evaluate whether a provider is appropriate for their needs.

3. Non‑Medical Use

The Platform is not intended for emergency, urgent medical, or clinical situations.

Clients should not use the Platform to request:

emergency medical assistance;

clinical treatment;

diagnosis or medical advice.

If a medical emergency occurs, clients should contact emergency services immediately.

4. Service Requests

Clients may submit service requests through the Platform.

Service requests may include details such as:

timing and scheduling

type of assistance requested

location of services

preferences or expectations

Clients should provide sufficient information for providers to evaluate whether they can safely accept the request.

5. Safety of the Environment

Clients are responsible for maintaining a safe environment for providers.

Clients should not request services in environments involving:

unsafe structural conditions

violent behavior

illegal activity

hazardous materials

Clients must disclose relevant safety concerns that could affect providers.

6. Respectful Conduct

Clients must treat providers with professionalism and respect.

Clients may not:

harass or discriminate against providers

threaten or intimidate providers

engage in abusive or unsafe conduct

Violations may result in suspension or termination of Platform access.

7. Payments

Clients are responsible for paying agreed service fees.

Payments may be processed through third‑party payment processors. Payment timing, refunds, cancellations, and related rules are governed by the HUB Payment Terms and Cancellation Policy.

Clients must ensure valid payment information is provided when required.

8. Cancellations and Scheduling

Clients may cancel or reschedule services subject to the Platform’s scheduling rules and cancellation policies.

Late cancellations or missed appointments may result in fees.

Repeated cancellations may result in account restrictions.

9. Provider Selection

Clients may review provider profiles when available.

Clients acknowledge that:

providers are independent contractors;

HUB does not guarantee provider performance;

background checks or verifications may be limited in scope.

Clients should exercise independent judgment when selecting providers.

10. Property and Personal Items

Clients are responsible for safeguarding valuables and personal property during services.

HUB is not responsible for loss, theft, or damage involving personal items unless required by law.

11. Disputes Between Users

If a dispute arises between a client and a provider, the parties should attempt to resolve the matter directly where appropriate.

HUB may review disputes but is not obligated to resolve them.

12. Reporting Concerns

Clients should report safety concerns, misconduct, or illegal activity encountered through the Platform.

HUB may review reports and take action where appropriate.

13. Suspension or Termination

HUB may suspend or terminate client access to the Platform if:

these Terms are violated

abusive behavior occurs

fraudulent activity is suspected

HUB is not obligated to continue providing access to the Platform.

14. Limitation of HUB Responsibility

HUB provides technology services only.

HUB is not responsible for:

provider conduct

service outcomes

disputes between users

conditions at service locations.

15. Changes to These Terms

HUB may update these Client Terms from time to time. Updated versions will be posted on the Platform with a revised effective date.

Continued use of the Platform constitutes acceptance of revised Terms.
